Locally advanced pancreatic body adenocarcinoma is a challenging operative problem given the propensity of the lesion to involve the major vascular structures. This video demonstrates the laparoscopic approach to a major vascular resection during pancreaticoduodenectomy, including resection and reconstruction of the superior mesenteric and portal veins. In expert hands this minimally invasive procedure is an exciting option for surgical management of tumor invasion in to critical structures.
