This video demonstrates a full-thickness hemi-circumferential excision of a uT2 rectal adenocarcinoma, status post neoadjuvant chemoradiation using Transanal Endoscopic Microsurgery in a morbidly obese female. The 3cm tumor was 10cm from the anorectal ring, necessitating sizable entrance into the peritoneal cavity to ensure complete full-thickness excision. Though the possible need for a stoma was considered and discussed with the patient, the large defect was closed primarily without proximal diversion. Patient had an uncomplicated post-operative course and was discharged from the hospital on post-operative day seven.
